About the role

Location: Remote (United States), multi-state licensure preferred.
Team: Clinical. Reports to: VP of Care Delivery. Employment: W2 or 1099. Role will be structured to fit how you want to work. Schedule: Flexible; 0.8, 0.9, and 1.0 FTE available. Ability to commit at least 20 hours per week. Ability to work up to 2 evening shifts and 2 weekend shifts per month. No overnights. Shift blocks (EST): 7:30a–1p, 1p–6p, 6p–9p.

What you'll do

This is a clinical role focused on direct patient care via messaging-based, AI-paired visits. You will spend the majority of your time seeing patients across acute, primary, and longevity care in an asynchronous messaging modality.
  • See patients over messages: diagnose, treat, triage, and follow up.
  • Practice with Counsel’s AI assistant that prepares history, sharpens differentials, and drafts messages while you provide clinical judgment.
  • Hold a high bar on clinical reasoning, safety, and patient communication.
  • Stay generalist: manage urgent care today, primary care tomorrow, and novel cases next week.
  • Collaborate with leadership on guidelines, peer review, and clinical systems to keep care safe as the team scales.
  • Mentor newer clinicians and contribute to raising clinical standards.
  • Provide feedback to shape the clinical tools and workflows you use.

What you bring

  • Board certification in Emergency Medicine, Internal Medicine, or Family Medicine and an active medical license (10+ licenses preferred; exceptional candidates with fewer will be considered).
  • 3+ years of post-residency clinical experience (urgent care, ER, primary care, or a strong generalist mix).
  • Genuine interest in acute, primary, and longevity care.
  • Comfort working in a fully virtual, asynchronous, messaging-based modality.
  • Strong written communication, as most patient care happens in writing.

Bonus points for

  • Multi-state licensure already in hand or a track record of standing up new licenses quickly.
  • Prior telehealth, virtual-first, or direct-to-consumer healthcare experience.
  • Experience incorporating longevity into clinical practice.
  • A clear point of view and experience using AI tools in clinical practice.
  • Background in fast-paced acute environments (ER, urgent care) with strong triage instincts.
