Opportunity
Join one of Twin Health's most critical member care management roles as a Registered Nurse. The RN will inspire behavior change and motivate members to adopt healthier behaviors to prevent and reverse chronic metabolic diseases. The role involves providing clinical guidance, support, education, and encouragement remotely.
This position suits those with skills in building rapport, motivation, teamwork across interdisciplinary teams, and leveraging data for clinical decision-making. The RN will work within a flexibly scheduled team pod structure and must adapt to dynamic member and team needs.
Responsibilities
- Deliver remote clinical monitoring and education via software, video conferencing, and electronic medical record
- Promote member self-care management using clinical judgment, data analysis, and critical thinking
- Evaluate member progress and barriers to plan of care and health status
- Manage members by monitoring health condition trends, lab reviews, and medication management
- Collaborate closely with nurses, health coaches, providers, and operations teams for seamless member experience
- Apply nursing philosophy holistically from intake to ongoing care
- Give appropriate health guidance using clinical knowledge and protocols
- Triage member alerts and inquiries using nursing processes
- Adhere strictly to SOPs for member management and escalations
- Provide timely responses and feedback to teammates on member care
- Support medication reconciliation and adherence, including working with pharmacies and members to reduce costs
- Maintain accurate and confidential records per federal laws and company policies
- Generate and analyze reports for management, identifying trends and concerns
- Contribute to clinical care improvements enhancing cost-effectiveness and quality
- Participate in ongoing education and performance improvement activities
- Perform additional duties as assigned
Qualifications
- Active, unencumbered RN license in the state of patient care
- Minimum 4 years nursing experience in varied healthcare settings like hospitals, FQHCs, ambulatory care, payor systems, etc.
- Preferred experience in case/care management, value-based care, population health, care coordination, or transition care management
- BSN or MSN from accredited nursing school required
- Preferred skills in motivational interviewing and behavior change
- Comfortable adopting new technologies and integrating data into patient care
- Ability to manage large caseloads and work effectively in fast-paced environments
- Proficient in multitasking with internet, electronic documentation systems, and business tools
- Comfortable with ambiguity and change
- Experience in high-growth or rapidly changing environments
- Professional telephone and video communication skills
- Self-motivated, results-focused, client service oriented, quick learner, organized, detail-oriented, diplomatic, and collaborative
- Passion for Twin's mission to transform lives by reversing, preventing, and improving chronic metabolic diseases
- Fluent bilingual in Spanish required
Compensation and Benefits
The compensation range for this position is $80,000 to $85,000 annually.
