Opportunity

The Nurse Practitioner role supports members in multiple states through remote care management, providing treatment, management, and guidance to members seeking diabetic reversal and health improvement across multiple conditions, using AI, machine learning, and a care model involving health coaches and providers.

Responsibilities

  • Engage with a collaborative team including health coaches, physicians, and medical officers
  • Conduct health assessments including labs, history, and psychosocial review
  • Assess symptoms and treat appropriately, collaborating with primary care providers
  • Deliver care using telemedicine and document in the clinical platform
  • Manage high risk populations with understanding of social determinants of health
  • Learn and support the Twin Model of Care for diabetes and chronic condition reversal
  • Understand business objectives and service level agreements supporting clinical and financial success
  • Collaborate with medical team to provide excellent member care service around chronic conditions
  • Provide patient education for disease prevention and health maintenance
  • Discuss and review medical history, symptoms, allergies, and medications
  • Assess symptoms through situation-specific questions
  • Prescribe medications and provide dosage instructions per policy
  • Maintain accurate records of patient contacts, history, prescribed medications and progress
  • Perform additional duties as assigned

Qualifications

  • Advanced Practice Registered Nurse license in multiple states
  • 3-5 years experience as advanced practice provider
  • Clinical experience with diverse and underserved populations
  • Experience managing remote patients across states
  • 5 or more licenses, willingness to obtain more
  • Board-certified in family or internal medicine
  • Proven advanced practice provider experience and sound medical knowledge
  • Ability to consult patients via virtual channels
  • Excellent analytical, problem-solving, and communication skills
  • Patient, compassionate, and detail-oriented
  • Interest in working with underserved groups
  • Experience using technology and data in care decisions
  • Fluent in English; Spanish preferred

Compensation and Benefits

The position offers an annual salary of $120,000.
