The Role

Behavioral Health Care Managers are central to the collaborative care model. As the main point of contact for each patient and their family, you will coordinate care and deliver evidence-based behavioral interventions to improve patient outcomes. You will use clinical screens, patient education, inquiry, support, and guided discovery to build awareness and responsibility, provide structure and resources, and facilitate access to experts as needed.

Responsibilities

Responsibilities include clinical screening, education, goal-setting, empathetic listening, behavioral interventions appropriate for pediatric primary care, and care coordination with the broader clinical team.
  • Screening: evaluate clinical screen results and adapt coaching approaches; use Triplemoon registry to measure impact over time.
  • Educating: host regular virtual one-on-one sessions with parents and/or pediatric patients to address behavioral health, wellbeing, and parenting.
  • Establishing goals: empower families to identify and achieve goals addressing family or parenting challenges.
  • Listening empathetically: listen actively and empathize with members.
  • Applying behavioral interventions: provide brief interventions such as CBT, Motivational Interviewing, Problem Solving, Behavioral Activation, or FAST.
  • Coordinating: facilitate communication with the care team, Triplemoon clinical operations, and primary care providers.

Qualifications

Required qualifications, skills, and characteristics include clinical experience with pediatric populations, strong communication and digital health skills, organization, and the ability to provide DEI- and trauma-informed care.
  • Relevant background as a licensed professional counselor, licensed therapist, and/or licensed social worker (prefer 2+ years experience).
  • Experience providing clinical intervention with children ages 0–17 across common pediatric diagnoses and partnering with parents.
  • Comfort with primary care pace and delivering brief evidence-based interventions and supporting medication management.
  • Willingness to develop knowledge of developmental pediatrics and learn brief evidence-based pediatric interventions.
  • Ability to balance patient and family needs while collaborating with other systems of care.
  • Deliver care using a diversity, equity, inclusion, and trauma-informed lens.
  • Strong digital health and telehealth skills; organized and punctual for logistics, scheduling, and follow-up.
  • Strong verbal and written communication and willingness to maintain accurate patient health records.

Licensure requirements

Active licensure in one or more of the following states on or before start date is required:
  • California
  • Texas
  • Maryland
  • Florida
  • Colorado
  • Tennessee

Preferred Skills (a plus, not required)

  • Spanish speaking
  • Experience working alongside medical teams and/or in primary care settings
  • Motivational interviewing training
  • PMADs recognition and intervention
  • Familiarity with CBT methodology

Compensation

The pay range for this role is 60,000 - 65,000 USD per year (Remote).
