Job Description

Helping patients feel supported, heard, and guided—right from your home.
As a Remote RN Case Manager, you will build trusted relationships with patients by phone, helping them navigate complex health needs, understand their care plans, and improve their overall well-being. This is a primarily remote role with 10–15% local travel to clinical sites in your hiring area.
  • Conduct in-depth telephonic assessments of medical, psychosocial, and social needs.
  • Review and update medical histories, including medications and preventive care.
  • Develop individualized care plans and guide patients through treatment goals and options.
  • Provide evidence-based education on chronic disease management and preventive health.
  • Monitor progress, adjust care plans, and ensure patients stay connected to providers.
  • Complete Medicare Annual Wellness Visits (AWVs) via telehealth under physician supervision.
  • Partner with Healthguides for non-clinical needs (scheduling, transportation, food assistance, SDOH resources).
  • Perform proactive outreach and timely follow-ups to maintain continuity of care.
  • Document accurately in the EHR and care-management systems.
  • Support quality outcomes (HEDIS, NCQA) and participate in virtual meetings and ongoing training.

Qualifications

What you'll need for success:
  • Active Compact Registered Nurse license in good standing.
  • 3+ years of RN case management experience, preferably in outpatient or ambulatory care.
  • 3+ years of RN case management experience specifically focused on complex pediatric cases.
  • Strong critical-thinking and problem-solving skills and ability to work independently.
  • Outstanding communication and telephonic interviewing skills, with accurate real-time documentation.
  • Comfortable with technology: EMRs, care-management systems, multi-screen workflows, Microsoft Office, and telecommunication tools.
  • Understanding of accreditation and quality standards (NCQA, HEDIS).
  • A quiet, private home office suitable for patient calls and handling PHI.

Additional Information & Benefits

The salary range for this role is $80,000.00 to $83,000.00, paid bi-weekly per standard payroll practices. Final base pay depends on factors such as skill set, experience, education, location, and licensure/certifications.
