Job Description
As a Healthguide you will strengthen the connection between patients and their primary care teams, help patients navigate care, solve healthcare issues, and improve communication with medical practices.
What you'll be doing
- Work primarily remotely to manage a targeted population of patients in coordination with medical practice staff and Guidehealth’s Case Management team.
- Function independently and collaboratively as an extension of the medical practice.
- Assist patients in achieving better health outcomes by addressing questions, connecting them with medical practices, implementing care plans, closing care gaps, escalating medical issues, and navigating referrals to specialty care and community-based organizations.
- Accurately document patients’ medical history and issues in the electronic health record (EHR).
- Understand the referral and prior authorization process and escalate appropriately to clinical staff when needed.
- Meet assigned Key Performance Indicators to improve patient outcomes and decrease medical spend.
- Use calls, texts, portals, and emails to engage and schedule high-risk patients.
- Follow security policies to protect PHI and Guidehealth intellectual property.
- Position may require compensated travel (10-15%) to clinical sites in the Atlanta, GA area for patient and provider engagement and conducting retinal eye exams with paid training provided.
Qualifications
What you'll need to have
- Certified or Registered Medical Assistant required with two or more years of experience.
- Must be bilingual English/Spanish.
- Experience and passion for patient coordination and serving people with complex medical and social needs.
- Willingness to learn motivational interviewing, critical listening, and behavioral change management techniques.
- Strong interpersonal skills with empathy, patience, professionalism, and respect.
- Exceptional documentation skills and ability to use software in real time.
- Familiarity with medical terminology and primary care practice workflows.
- Knowledge of social determinants of health and experience with various EHRs (eClinicalWorks, EPIC, Athena, Cerner).
- Proficiency in Microsoft Office (Outlook, Word, Excel) and ability to navigate multiple technology platforms.
- Reliable transportation and a valid driver’s license for occasional travel to provider sites; ability to maintain patient confidentiality in a private home office.
Additional Information
The base pay range for this role is between $21-$24 per hour paid bi-weekly.
Guidehealth is a fully remote company.
