Job Description
As a Healthguide, you will be supported by clinical teams in strengthening the connection between the patient and their primary care physician/medical practice staff, and improving the patient’s health and well-being. The Healthguide engages patients, helps them navigate their care, resolves healthcare issues, and improves communication with their medical practice.
WHAT YOU’LL BE DOING
- Work primarily remotely with patient medical practice staff and Guidehealth’s Case Management team to manage a targeted patient population.
- Function independently and collaborate as an extension of the medical practice.
- Assist patients in achieving better health outcomes by addressing questions and concerns, connecting them with the medical practice, implementing care plans, closing care gaps, escalating medical issues, and navigating referrals to specialty care and community-based organizations.
- Accurately document patient medical history and issues in the electronic health record (EHR).
- Understand referral and prior authorization processes.
- Build trusting professional relationships with assigned medical practices and within Guidehealth.
- Meet assigned Key Performance Indicators to improve patient outcomes while decreasing medical spend.
- Use calls, texts, portals, and emails to engage and schedule high-risk patients.
- Escalate medical issues to the Healthguide Registered Nurse, primary care physician, or Guidehealth leadership as appropriate.
- Follow security policies and procedures to protect PHI and Guidehealth intellectual property.
- Position may require travel (~10-15%) to clinical sites in the Atlanta, GA area for patient/provider engagement and retinal eye exams (paid training provided).
Qualifications
WHAT YOU'LL NEED TO HAVE
- Certified or Registered Medical Assistant required with 2+ years of experience.
- Must be bilingual (English/Spanish).
- Passion for patient coordination for people with complex medical and social needs.
- Willingness to learn motivational interviewing, critical listening, and behavioral change management.
- Strong interpersonal skills: empathy, patience, professionalism, respect.
- Excellent conversational and documentation skills, documenting interviews in real time.
- Experience with medical terminology and primary care practice workflows.
- Knowledge of social determinants of health.
- Ability to navigate multiple technologies and platforms (EHRs, analytics databases, dialing systems, Microsoft Office).
- Proficiency in Microsoft Outlook, Word, Excel.
- Familiarity with EHRs such as eClinicalWorks, EPIC, Athena, Cerner.
- Ability to balance competing priorities while detail-oriented.
- Ability to work independently under supervision of licensed clinical professionals.
- Proven success working in a remote environment.
- Reliable transportation and valid state driver’s license for occasional travel to provider sites.
- Maintain patient confidentiality using a provided headset in a private home office.
- Other duties and compliance with Guidehealth policies as assigned.
Additional Information
The base pay range for this role is between $21-$24 per hour paid bi-weekly

COMPENSATION
Listed compensation range is paid bi-weekly. Final base pay depends on factors including experience, education, location, and certifications.

REMOTE WORK TECHNICAL REQUIREMENTS
Guidehealth provides necessary equipment at no charge. Employees must provide their own internet capable of video calls with minimum speeds of 100 Mbps download and 10 Mbps upload.
