Job Description
As a Healthguide, you work remotely supporting connection between patients and their primary care physicians/medical practice staff to improve patient health and well-being. You engage patients, help navigate care, solve healthcare issues, and improve communication with their medical practice.
- Work primarily remotely with medical practice staff and Guidehealth's Case Management team to manage a targeted patient population.
- Function independently and collaborate as an extension of the medical practice.
- Assist patients in achieving better health outcomes by addressing questions, connecting them with medical practice, implementing care plans, closing care gaps, escalating medical issues, and referring to specialty care and community organizations.
- Document patients' medical history and issues accurately in electronic health records (EHR).
- Understand referral and prior authorization processes.
- Build professional relationships with medical practices and Guidehealth organization.
- Meet assigned Key Performance Indicators for positive patient outcomes while reducing medical spend.
- Use communication modalities (calls, texts, portals, emails) to engage and schedule high-risk patients.
- Escalate medical issues appropriately to registered nurses, physicians, or Guidehealth leadership.
- Follow security policies to protect all PHI and Guidehealth intellectual properties.
- Position requires compensated travel (10-15%) to clinical sites in Columbus, GA for patient/provider engagement and retinal eye exam duties with training provided.
- Applicants must live within 60 miles of Columbus, GA.
Qualifications
- Certified or Registered Medical Assistant OR Certified EMT with 2+ years experience.
- Passion for patient coordination for complex medical and social needs.
- Willingness to learn motivational interviewing, critical listening, and behavioral change techniques.
- Strong interpersonal skills: empathy, patience, professionalism, respect.
- Excellent conversation skills and accurate patient interview documentation using software.
- Understanding of medical terminology and primary care physician function.
- Knowledge of social determinants of health.
- Ability to independently navigate technology: Electronic Medical Records, databases, phone systems, Microsoft Office.
- Proficient in Microsoft Office (Outlook, Word, Excel).
- Familiarity with EHR systems like eClinicalWorks, EPIC, Athena, Cerner.
- Detail-oriented with ability to manage competing priorities.
- Excellent communication skills (written, visual, verbal).
- Ability to track multiple tasks to completion.
- Ability to work independently under licensed clinical supervision.
- Experience with remote work environments.
- Reliable transportation and valid driver's license for occasional travel.
- Adherence to patient confidentiality using provided headset during private conversations.
- Compliance with Guidehealth policies.
Additional Information
Base pay range: $21-$24 per hour, paid bi-weekly.
