As a Healthguide, you will be supported by clinical teams to strengthen the connection between patients and their primary care physicians/medical practice staff, improving patients' health and well-being. You will engage patients, help navigate their care, solve healthcare issues, and improve communication with medical practices.

Responsibilities include working primarily remotely, collaborating with medical practice staff and case management teams, managing targeted patient populations, assisting patients in achieving better health outcomes, documenting patient history accurately, understanding referral and prior authorization processes, building professional relationships, meeting key performance indicators, utilizing various communication methods, appropriately escalating medical issues, following security policies, and occasional compensated travel to clinical sites for patient and provider engagement and retinal eye exams.

Certified or Registered Medical Assistant OR Certified EMT with 2+ years experience required. Bilingual in English and Spanish. Must have a love for patient coordination and proficiency in motivational interviewing, critical listening, and behavioral change management. Strong interpersonal, conversational, and documentation skills needed. Experience with medical terminology and primary care practice functions required. Knowledge of social determinants of health, tech competency with electronic medical records, care management databases, and Microsoft Office mandatory. Ability to work independently and remotely in a professional manner. Reliable transportation and valid driver's license for occasional travel required. Commitment to patient confidentiality and compliance with Guidehealth policies essential.

Base pay range between $21-$24 per hour, paid bi-weekly.
