The Manager, Clinical Care is responsible for day-to-day operations of a multidisciplinary clinical and support structure focused on value-based clinical initiatives, including case management, transitions of care, care coordination, and provider education. Strong leadership, management, and interpersonal skills are key, along with resource utilization to meet organizational goals.

Key responsibilities include managing clinical programs, improving outpatient utilization, implementing population health programs, executing case management for high-risk patients to reduce emergency visits/hospitalizations, overseeing patient care teams, ensuring quality measures, directing health education activities, participating in committees and meetings, monitoring initiatives, managing ACO program integration, maintaining business partnerships, implementing organizational changes, managing staff hiring and performance, prioritizing tasks and projects, representing health plans in audits, overseeing projects and productivity, ensuring EMR system access, monitoring HIPPA compliance, communicating with health plans and clients, and disseminating health plan announcements.

Requirements include RN/BSN, 5 years management experience, 4 years clinical nursing experience, 4 years chronic care case management in outpatient settings, preferred certification in case management, ability to work remotely and obtain licensure in IL, GA, PA, TX, confidence in leading certified and unlicensed team members, and strong analytical and communication skills.

The salary is around $110,000.
