About the role (What you'll be doing)
Through telehealth, you will conduct consultations with patients, including diagnosing and prescribing medication as appropriate and managing medication side effects. You will provide exceptional ongoing clinical support and respond to medical issues, and assist in creating improved models of care and extensive support programs to drive better patient outcomes.
- Conduct telehealth consultations, diagnose and prescribe medication where appropriate and manage medication side effects
- Provide ongoing clinical support for patients
- Respond to medical issues and escalations in a timely manner
- Contribute to the development of improved models of care and patient support programs
About you (Who you are)
We are looking for an experienced, patient-centred Nurse Practitioner who is comfortable working in a digital/telehealth environment and making timely clinical decisions.
- Qualified health professional: you are a qualified Nurse Practitioner
- Passion for telehealth and primary care: demonstrated experience in primary care conditions; telehealth experience is a plus
- Tech-savvy: confident navigating digital applications for clinical practice
- Experienced decision maker: able to make timely, accurate referral and clinical decisions under pressure
- Trustworthy and clear communicator: patients and colleagues trust your experience and clarity
- Compassionate: patients feel safe, supported and valued in your care
