Virtual Oncology Nurse Navigator

ABOUT THE ROLE

As an Oncology Nurse Navigator at Daymark Health, you will be a key member of an integrated Care Team, which includes a Medical Director, Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker. You will work closely with the Clinical Operations team to ensure optimal care pathways are identified and followed. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities. You will deliver care by phone and through virtual platforms to improve patients' quality of life.

WHAT YOU'LL DO

In this role, you will provide direct patient care through virtual platforms and deliver high-quality care to meet each patient's unique needs. You will provide comprehensive support throughout the oncology care journey, from screening to survivorship or end-of-life care, offering clinical and care coordination as needed.
  • Clinical Support: Provide consultative support by phone, email, or video and be available for urgent clinical escalations when required.
  • Symptom Triage: Triage responses to symptom surveys and inbound reported symptoms, support the patient when possible and escalate to a Nurse Practitioner as needed.
  • Care Navigation: Proactively identify and address patient needs including care coordination, nutritional support, discharge planning, and provider referrals.
  • Patient Education: Help patients understand their diagnosis, Daymark Health’s care offerings, and recommended next steps.
  • Relationship Building: Establish strong, trusting relationships with patients and their care networks through empathy and active listening.
  • Integrated Care Coordination: Collaborate with clinical team members and non-clinical health partners to address clinical protocols and social determinants of health.
  • Operational Excellence: Adhere to Daymark’s policies, procedures, and documentation standards and collaborate with Clinical Operations to use systems and tools effectively.

WHAT WE ARE LOOKING FOR

  • Experience: Bachelor of Science in Nursing required, compact unrestricted RN license (or ability to obtain) and willingness to be licensed in Rhode Island. Minimum 3 years of nursing experience, including at least 2 years in oncology nursing. Interest in palliative care.
  • Qualifications: Oncology-related certifications (OCN, AOCN, AOCNS, or CCM) strongly preferred or to be obtained within two years of hire.
  • Comfort with Technology: Comfortable using Slack, Zoom, Google Suite, and EHR systems.
  • Relational Skills: Ability to establish rapport with diverse populations including providers, clinical staff, patients, and families.
  • Bias to Action: Proactive, able to prioritize and act on urgent needs without waiting for direction.
  • Patient-Centric Approach: Prioritize the patient experience and demonstrate commitment to Daymark's mission.
  • Organizational Skills: Excellent multitasking and organization in fast-paced environments.
  • Communication Skills: Effective listener and communicator, skilled at building rapport and relationships.
  • Adaptability: Comfortable with change and ambiguity and successful in dynamic environments.

ADDITIONAL INFORMATION

This is a full time, remote role. Hours are Monday through Friday from 8:30 AM to 5:00 PM Eastern Time.
Compensation is anticipated to be in the range of $85,000 to $100,000 annually, depending on location and experience.
