ABOUT THE ROLE

As a Registered Nurse at Daymark Health, you will be a key member of an integrated Care Team, which includes a Nurse Practitioner, Health Partner, Enrollment Specialist, and Social Worker. You will deliver hands-on, compassionate care directly in patients’ homes and through virtual platforms. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities.
Your primary focus is delivering direct clinical care in the home: administering treatments, managing symptoms, conducting assessments, and being a trusted, present resource for patients and their families. You will also support patients through virtual platforms and help them access Daymark’s services and support.

WHAT YOU'LL DO

Provide direct patient care in the home and through virtual platforms, conduct health assessments, educate patients, and deliver high-quality care tailored to each patient's needs. Key responsibilities include:
  • Deliver In-Home Clinical Care. Administer IV fluids, access ports, perform point-of-care testing, and manage acute and chronic symptoms in patients' homes.
  • Provide Urgent Clinical Support. Triage urgent needs, escalate to home visits or Nurse Practitioners, offer consultative clinical guidance, and help patients navigate critical moments.
  • Support Patients Across the Full Care Continuum. Provide clinical support from active treatment through survivorship or end-of-life care.
  • Proactively Address Patient Needs. Identify and act on symptom management, nutritional support, discharge planning, and provider referrals.
  • Address Social Determinants of Health. Collaborate with non-clinical Care Team members to resolve barriers like food insecurity and transportation.
  • Master Daymark Systems. Learn and use Daymark's systems, tools, technology, and partners.
  • Build Trusting Relationships. Establish empathetic, respectful relationships with patients and care networks.
  • Adhere to Operational Standards. Follow Care Team policies, procedures, and documentation standards to maintain high-quality care.

WHAT WE ARE LOOKING FOR

  • Experience: Bachelor of Science in Nursing required. Compact unrestricted RN licensure in Rhode Island required. At least 3 years of nursing experience; oncology nursing and/or home care experience strongly preferred. Passion for palliative care is important.
  • Availability: Full-time, five days per week.
  • Qualifications: Oncology-related certifications (OCN, AOCN, AOCNS) or Certified Case Manager (CCM) are a plus.
  • Strong Clinical Skills: Comfortable administering IV fluids, accessing ports, and conducting point-of-care testing.
  • In-Home Care Competency: Able to work independently in patients’ homes and exercise strong clinical judgment.
  • Comfort with Technology: Familiarity with Slack, Zoom, Google Suite, and EHR systems.
  • Relational and Communication Skills: Ability to build rapport with diverse populations and communicate effectively with patients and colleagues.
  • Bias to Action and Organizational Skills: Proactive, able to prioritize urgent needs, multitask, and maintain organized documentation.
  • Adaptability and Patient-Centric Approach: Comfortable with change and committed to prioritizing the patient experience and Daymark's mission.

Additional Information

The expected compensation for this role is $90,000 to $110,000 annually, depending on location and experience.
Office is based in Providence, RI and this role delivers in-home care to patients in Rhode Island. Candidates must have a valid driver's license and a reliable personal vehicle.
