Your Role: Outreach & Engagement Liaison
As an Outreach Liaison, you will be a frontline ambassador for Daymark Health19s building trust with healthcare providers, enrolling new patients, and helping individuals access compassionate, team-based care. You19ll spend time both in the field and in the office: introducing our services to local providers and specialists, supporting patient referrals and enrollment, and delivering an exceptional first experience to those joining Daymark19s program.
What You19ll Do
Provider Outreach & Referral Support
  • Develop and maintain strong relationships with local providers, specialists, and their office staff across Rhode Island.
  • Educate providers and staff about Daymark19s services, referral workflows, and impact on patient outcomes.
  • Represent Daymark at medical events, health fairs, and industry meetings to grow awareness.
  • Monitor referral activity and troubleshoot barriers to ensure patients are connected efficiently.
  • Collaborate with our clinical and operational teams to align on outreach, referrals, and enrollment strategy.
Patient Enrollment & Engagement
  • Make 50+ outbound calls per day to prospective patients and caregivers to introduce Daymark19s services and support.
  • Guide patients through our enrollment process, surfacing goals and concerns to help tailor support.
  • Respond to inbound communications, triage requests, and ensure patients are connected to the right team members.
  • Obtain patient consent, complete enrollment surveys, and escalate urgent needs to our clinical teams.
  • Share feedback with internal teams based on patient and provider needs, referral trends, and barriers to access.

  • You19ve built trust with local provider offices and have established a steady referral pipeline. You consistently meet or exceed daily outreach and enrollment goals.
  • You19ve become a go-to resource for both patients and providers, known for your professionalism, warmth, and follow-through.
  • You19ve contributed to improvements in our enrollment process and patient experience by identifying gaps and opportunities.
What leads to success
  • Sales/Enrollment experience - you have 2+ years of experience in an enrollment or sales role and are motivated to help patients access better care and hit enrollment targets.
  • Relational & Empathetic: You know how to build trust19 quickly and genuinely19 with both providers and patients. You lead with curiosity, empathy, and clarity.
  • Organized & Proactive: You take initiative, stay on top of details, and bring structure to dynamic or ambiguous situations.
  • Strong Communicator: You write and speak clearly and professionally. You tailor your communication to fit your audience and situation.
  • Tech-Savvy: You19re comfortable with tools like Slack, Salesforce, Zoom, Google Suite, and EHRs. You adapt easily to new systems and workflows.
  • Based in Rhode Island: You19re available to work regularly from our local office and travel to provider sites across the region.
  • Reliable transportation: You must have reliable transportation for off-site meetings.

All roles that require in-home visits to patients or to provider offices must provide proof of a valid driver's license.
This role will be compensated with a base + commission structure. We anticipate the base to be $50,000-$65,000 with an opportunity for a $15,000-$20,000 commission, should you hit performance and engagement targets.
