Role: Outreach & Engagement Liaison
As an Outreach Liaison, you will be a frontline ambassador for Daymark Health—building trust with healthcare providers, enrolling new patients, and helping individuals access compassionate, team-based care. You’ll spend time both in the field and in the office: introducing our services to local providers and specialists, supporting patient referrals and enrollment, and delivering an exceptional first experience to those joining Daymark’s program.
What You'll Do
Provider Outreach & Referral Support
- Develop and maintain strong relationships with local providers, specialists, and their office staff across Rhode Island.
- Educate providers and staff about Daymark’s services, referral workflows, and impact on patient outcomes.
- Represent Daymark at medical events, health fairs, and industry meetings to grow awareness.
- Monitor referral activity and troubleshoot barriers to ensure patients are connected efficiently.
- Collaborate with our clinical and operational teams to align on outreach, referrals, and enrollment strategy.
Patient Enrollment & Engagement
- Make 50+ outbound calls per day to prospective patients and caregivers to introduce Daymark’s services and support.
- Guide patients through our enrollment process, surfacing goals and concerns to help tailor support.
- Respond to inbound communications, triage requests, and ensure patients are connected to the right team members.
- Obtain patient consent, complete enrollment surveys, and escalate urgent needs to our clinical teams.
- Share feedback with internal teams based on patient and provider needs, referral trends, and barriers to access.
Success in 3 Months
- Build trust with local provider offices and establish a steady referral pipeline.
- Consistently meet or exceed daily outreach and enrollment goals.
- Become a go-to resource for both patients and providers, known for professionalism, warmth, and follow-through.
- Contribute to improvements in enrollment process and patient experience by identifying gaps and opportunities.
Success Factors
- 2+ years experience in enrollment or sales roles with motivation to help patients access better care and achieve targets.
- Relational and empathetic approach to build trust quickly and genuinely with providers and patients.
- Organized and proactive, taking initiative and bringing structure to dynamic or ambiguous situations.
- Strong communication skills, writing and speaking clearly and professionally tailored to audience.
- Comfort with tools like Slack, Salesforce, Zoom, Google Suite, and EHRs, adapting to new systems and workflows.
- Based in Rhode Island with availability to work locally and travel to provider sites across the region.
- Must have reliable transportation for off-site meetings.

Compensation
This role offers a base plus commission structure, anticipating a base salary of $50,000-$65,000 with potential commission of $15,000-$20,000 based on performance and engagement targets.
