ABOUT THE ROLE
As an Oncology Nurse Navigator at Daymark Health, you will be a key member of an integrated Care Team that includes a Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker. You will work closely with the Clinical Operations team to ensure optimal care pathways are identified and followed. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities. Your role is essential in improving patients' quality of life by delivering care both in patients' homes and through virtual platforms.
WHAT YOU'LL DO
Provide direct patient care in the home and through virtual platforms. Conduct health assessments, educate patients, and deliver high-quality care tailored to each patient's needs.
Within your first three months, you will:
- Master Daymark systems and tools, including Daymark's systems, technology, and partners.
- Collaborate closely with Clinical Leadership to align with clinical protocols and best practices.
- Build trusting relationships with patients and their care networks through empathy and active listening.
- Adhere to operational standards, policies, procedures, and documentation expectations.
- Support patients across the care continuum from screening to survivorship or end-of-life care, providing clinical and care coordination as needed.
- Proactively address patient needs including care coordination, symptom management, nutritional support, discharge planning, and provider referrals.
- Contribute to case conferences to monitor patient progress and collaborate on support plans.
- Strengthen payer and provider relationships to optimize care delivery and reduce readmission risk.
- Address social determinants of health by collaborating with non-clinical team members on issues like food security and transportation access.
- Provide urgent clinical support and consultative clinical assistance when required.
WHAT WE ARE LOOKING FOR
- Experience: Bachelor of Science in Nursing required, compact unrestricted RN license and licensure in Rhode Island (or willingness to obtain RI license). Minimum of 3 years nursing experience including 3 years in oncology nursing, with significant in-home care experience. Passion for helping patients understand the benefits of palliative care.
- Availability: Able to work full-time from the Providence, Rhode Island office five days per week.
- Qualifications: Oncology-related certifications (OCN, AOCN, AOCNS, or CCM) strongly preferred or to be obtained within 2 years of hire.
- Clinical skills: Comfortable administering IV fluids, accessing ports, and performing point-of-care testing.
- Comfort with technology: Proficient with Slack, Zoom, Microsoft Office, and EHR systems.
- Relational skills: Ability to establish rapport with diverse populations including providers, clinical staff, patients, and families.
- Bias to action: Able to identify priorities and act promptly on urgent and important needs.
- Patient-centric approach: Prioritizes patient experience and Daymark's mission.
- Organizational skills: Strong multitasking ability and meticulous documentation.
- Communication skills: Effective listener and communicator.
- Adaptability: Comfortable with change and ambiguity in dynamic environments.
- Willingness to travel: Able to travel to patient homes in Rhode Island.

Compensation Range: $85,000 - $100,000 USD
