Oncology Nurse Navigator — Daymark Health
Daymark Health is a value-based oncology company redefining the cancer care experience for patients, providers, and health plans. The Oncology Nurse Navigator is a core member of an integrated Care Team and provides triage, case management, care navigation, and direct patient care in patients' homes and via virtual platforms to improve quality of life and care coordination.
About the role
As an Oncology Nurse Navigator you will work with a Nurse Practitioner, Community Health Worker, Enrollment Specialist, Social Worker, and Clinical Operations to identify and follow optimal care pathways. You will support oncology patients — many with comorbidities — by delivering clinical care, symptom management, education, and coordination across the care continuum.
What youll do
You will provide direct patient care in the home and virtually, perform health assessments, educate patients, and deliver individualized, high-quality care. Responsibilities include:
- Master Daymark systems, tools, technology, and partners.
- Collaborate with Clinical Leadership to ensure alignment with clinical protocols and best practices.
- Establish trusting relationships with patients and their care networks through empathy and active listening.
- Follow Care Team policies, procedures, and documentation standards.
- Support patients across the oncology care journey from screening to survivorship or end-of-life care.
- Identify and address patient needs such as care coordination, symptom management, nutritional support, discharge planning, and provider referrals.
- Participate in case conferences to monitor progress and collaborate on targeted support plans.
- Build partnerships with payers and providers to optimize care delivery and reduce readmissions.
- Work with non-clinical team members to address social determinants of health (e.g., food security, transportation).
- Provide urgent clinical support and consultative clinical assistance when required.
Qualifications & additional information
Required: Bachelor of Science in Nursing (BSN); compact unrestricted RN license and licensure in Rhode Island (or willingness to obtain RI license); at least 3 years of nursing experience including 3 years in oncology nursing; experience delivering in-home care. Must be able to work full-time from Daymark's Providence, Rhode Island office five days per week and be willing to travel in-home to care for patients in Rhode Island.
Preferred or to be obtained within two years: oncology-related certifications such as OCN, AOCN, AOCNS, or Certified Case Manager (CCM). Strong clinical skills (IV fluids, port access, point-of-care testing), comfort with technology (Slack, Zoom, Microsoft Office, EHR), excellent communication and relational skills, organization, adaptability, and a patient-centric approach are required.
All patient-facing roles require Basic Life Support (BLS) certification and compliance with Daymark's Employee Vaccination and Testing policy. Roles requiring in-home visits must provide proof of a valid driver's license.

Compensation Range: $85,000 - $100,000 USD
