About the Role
As an Oncology Nurse Navigator, you will be a key member of an integrated Care Team that includes a Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker. You will work closely with Clinical Operations to identify and follow optimal care pathways, provide triage, case management, and care navigation for oncology patients, many with comorbidities, and deliver care in patients' homes and via virtual platforms.
What Youll Do
Provide direct patient care in the home and through virtual platforms, conduct health assessments, educate patients, and deliver high-quality, individualized care.
Within the first three months you will:
- Master Daymark systems, tools, technology, and partners.
- Collaborate with Nurse Leaders and Medical Directors to align with clinical protocols and best practices.
- Establish strong, trusting relationships with patients and their care networks.
- Follow Care Team policies, procedures, and documentation standards.
- Provide support across the oncology care continuum from screening to survivorship or end-of-life care.
- Quickly identify and address patient needs including symptom management, discharge planning, and provider referrals.
- Participate in case conferences and collaborate on patient support plans.
- Build partnerships with payers and providers to optimize care and reduce readmissions.
- Address social determinants of health with non-clinical team members.
- Provide urgent clinical escalations and consultative clinical support when required.
Qualifications and Requirements
Required: Bachelor of Science in Nursing, an unrestricted compact RN license and licensure in Rhode Island (or willingness to obtain RI licensure), at least 3 years of nursing experience including 3 years in oncology nursing, and significant experience delivering in-home care. Candidates must be willing to travel to patient homes in Rhode Island and can work full-time from the Providence, Rhode Island office five days per week.
Preferred or to be obtained within 2 years: Oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM).
Clinical skills: comfortable administering IV fluids, accessing ports, and performing point of care testing. Comfortable using Slack, Zoom, Microsoft Office, and EHR systems. Strong relational, organizational, communication, and adaptability skills, with a bias to action and a patient-centric approach.
All patient-facing roles require Basic Life Support (BLS) certification and compliance with Daymark's Employee Vaccination and Testing policy; candidates without these at hire should coordinate with their recruiter to meet requirements by their start date. In-home roles require proof of a valid driver's license.
