About the Role

As an Oncology Nurse Navigator at Daymark Health, you will be a key member of an integrated Care Team, which includes a Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker. You will work closely with the Clinical Operations team to ensure that optimal care pathways are identified and followed. You will provide triage, case management, and care navigation for oncology patients, many of whom have additional comorbidities. Your role is essential in improving patients' quality of life by delivering care both in their homes and through virtual platforms.
What you’ll do
In this role, you will provide direct patient care in the home and through virtual platforms. Additionally, you will conduct health assessments, educate patients, and deliver high-quality care to meet each patient's unique needs.
Within your first three months, you will:
  • Master Daymark systems and tools. Become familiar with Daymark's systems, tools, technology, and partners.
  • Collaborate closely with Clinical Leadership. Work closely with Nurse Leaders and Medical Directors to ensure alignment with clinical protocols and best practices.
  • Build trusting relationships. Establish strong, trusting relationships with patients and their care networks by prioritizing empathy and active listening in every interaction.
  • Adhere to operational standards. Follow Care Team policies, procedures, and documentation standards to contribute to operational efficiency and maintain high-quality care.
  • Support patients across the care continuum. Provide comprehensive support throughout the oncology care journey, from screening to survivorship or end-of-life care, offering clinical and care coordination as needed.
  • Proactively address patient needs. Identify and address patient needs, including care coordination, symptom management, nutritional support, discharge planning, and provider referrals.
  • Contribute to case conferences. Participate in case conferences to monitor patient progress, provide updates, and collaborate with the healthcare team on targeted support plans.
  • Strengthen payer and provider relationships. Build strong partnerships with payers and providers to optimize care delivery and reduce readmission risk.
  • Address social determinants of health. Collaborate with non-clinical Care Team members to resolve issues such as food security and transportation access.
  • Provide urgent clinical support. Be available for urgent clinical escalations and offer consultative clinical support when required.

Qualifications

  • Experience: A Bachelor of Science Degree in Nursing is required, along with a compact unrestricted registered nurse (RN) license and licensure in Rhode Island (or willingness to obtain RI licensure). Additionally, at least 3 years of nursing experience, including 3 years in oncology nursing, and significant experience delivering in-home care is required. A passion for helping patients understand the benefits of palliative care is preferred.
  • Availability: Able to work full-time from the office in Providence, Rhode Island, five days per week.
  • Qualifications: Oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) are strongly preferred or will be obtained within 2 years of hire.
  • Strong Clinical Skills: Comfortable administering IV fluids, accessing ports when needed, and conducting point-of-care testing.
  • Comfort with Technology: Comfortable using Slack, Zoom, Microsoft Office, and EHR systems.
  • Strong Relational Skills: Ability to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.
  • Bias to Action: Proactively identify and prioritize needs and act on urgent matters immediately.
  • Patient-Centric Approach: Prioritize the patient experience and demonstrate commitment to Daymark's mission.
  • Organizational Skills: Excel at multitasking and thrive in fast-paced environments while maintaining meticulous organization in communications and documentation.
  • Communication Skills: Effective listener and communicator, skilled at building rapport and fostering strong working relationships with patients and colleagues.
  • Adaptability: Comfortable with change and ambiguity and proven success in dynamic environments.
  • Willingness to travel in-home to care for patients in Rhode Island.

Compensation Range: $85,000 - $100,000 USD
