About the Role
As an Oncology Nurse Navigator at Daymark Health, you will be a key member of an integrated Care Team including a Nurse Practitioner, Community Health Worker, Enrollment Specialist, and Social Worker. You will collaborate closely with the Clinical Operations team to ensure optimal care pathways for oncology patients, many with additional comorbidities.
Your responsibilities include triage, case management, and care navigation, primarily delivered over the phone or virtual platforms. You will also help patients enroll in Daymark's services and support.
Responsibilities
Provide direct patient care at home and virtually, conduct health assessments, educate patients, and deliver high-quality care tailored to individual patient needs.
Within the first three months, you will:
- Master Daymark systems and tools.
- Collaborate closely with Clinical Leadership to ensure alignment with clinical protocols.
- Provide urgent clinical support over the phone, email, or video, including during patient enrollment.
- Build trusting relationships with patients and care networks by prioritizing empathy and active listening.
- Adhere to operational standards for policies, procedures, and documentation.
- Support patients across the oncology care continuum from screening to survivorship or end-of-life care.
- Proactively address patient needs including care coordination, symptom management, nutritional support, discharge planning, and provider referrals.
- Participate in case conferences to monitor patient progress and collaborate on support plans.
- Address social determinants of health in collaboration with non-clinical Care Team members.
Qualifications
- Bachelor of Science in Nursing required, with a compact unrestricted RN license and Rhode Island licensure (or willingness to become licensed in RI).
- At least 3 years of nursing experience including 2 years in oncology nursing and significant in-home care delivery experience.
- Oncology-related certifications (e.g., OCN, AOCN, AOCNS, CCM) strongly preferred or to be obtained within 2 years of hire.
- Strong clinical skills including IV fluid administration, accessing ports, and point of care testing.
- Comfort with technology like Slack, Zoom, Microsoft Office, and EHR systems.
- Strong relational and communication skills, able to work with diverse populations.
- Bias to action, patient-centric approach, strong organizational skills, and adaptability in dynamic environments.
- Comfort with occasional in-home care visits and valid driver's license proof required.

Compensation
Salary range: $85,000 - $100,000 USD
