About the role
Medical Assistants are core members of the Daymark care team and support integrated, whole-person care for patients. In this role you will coordinate social and logistical aspects of care, connect with patients and caregivers by phone, video, and occasionally in the home, help patients navigate community-based and social services, ensure follow-up appointments are scheduled, and support coordination between Daymark and external providers.
What you will do
Key responsibilities include:
- Conduct structured social needs assessments (SDOH) and connect patients with community resources such as transportation, financial assistance, food, medical equipment, or emotional support programs.
- Support development of patients’ care plans and participate in case conferences to ensure quality care delivery.
- Serve as the primary point of contact for virtual visit readiness and basic technology troubleshooting.
- Coordinate and schedule care for patients, including scheduling follow-up visits and facilitating referrals.
- Prepare charts to ensure providers are visit-ready, including accurate contact, insurance, and care team information.
- Track completion of Symptom Surveys, complete appropriate follow ups, and escalate clinical concerns as needed.
- Document social interventions, tasks, and care coordination activities in the electronic health record in a timely and standardized format.
Qualifications & additional information
Experience: At least 3 years of experience as a Medical Assistant, CNA, EMT, or in a similar clinical or care coordination setting. Oncology experience is a bonus. Candidates must be comfortable driving to visit patients in the field.
Technology: Comfortable using Slack, Zoom, the Google Suite, and EHR systems to manage scheduling and support patients virtually.
Other: Candidates should demonstrate a bias to action, comfort with change and ambiguity, strong relational skills, and be organized and communicative. Ability to work from Daymark's office in Providence, Rhode Island five days per week and visit patients' homes as needed is required. Bilingual skills are a plus but not required.
Compliance: All patient-facing roles require Basic Life Support (BLS) certification and compliance with Daymark's Employee Vaccination and Testing policy. Roles requiring in-home visits must provide proof of a valid driver's license.

Compensation Range: $50,000 - $60,000 USD
