THE ROLE
Medical Assistants are core members of the Daymark care team, supporting integrated, whole-person care for our patients. In this role you will coordinate social and logistical aspects of care, connecting with patients and caregivers by phone, video, and occasionally in the home. You will help patients navigate community-based and social services, ensure follow-up appointments are scheduled, and support coordination between Daymark and external providers.
WHAT YOU'LL DO
- Conduct structured social needs assessments (SDOH) and connect patients with community resources, which may include transportation, financial assistance, food, medical equipment, or emotional support programs.
- Support development of patients’ care plans and participate in case conferences to ensure quality care delivery.
- Serve as the primary point of contact for virtual visit readiness and basic technology troubleshooting.
- Coordinate and schedule care for patients, including scheduling follow-up visits and facilitating referrals.
- Own structured chart preparation to ensure providers are visit-ready, including accurate contact, insurance, and care team information.
- Track completion of our Symptom Survey, complete all appropriate follow ups, and escalate clinical concerns as needed.
- Document social interventions, tasks, and care coordination activities in the electronic health record in a timely and standardized format.
WHAT WE ARE LOOKING FOR
- Experience. At least 3 years of experience as a Medical Assistant, CNA, EMT, or in a similar clinical or care coordination setting. Oncology experience is a bonus. Comfortable driving to visit patients in the field.
- Comfort with Technology. Comfortable using Slack, Zoom, the Google Suite, and EHR systems to ensure accurate appointment scheduling and management and support patients virtually.
- Bias to action. Proactive and able to identify and prioritize needs and act on urgent matters immediately.
- Comfort with change and ambiguity. Able to work without always having explicit direction and determine a path forward.
- Strong relational skills. Able to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.
- Organized & Communicative. Highly organized, detail-oriented, and proficient in collecting patient clinical and demographic data and documenting appropriately in a timely manner.
- Open to working from Daymark's office. Comfortable working out of the Providence, Rhode Island office five days per week and visiting patients' homes when needed.
- Bilingual. A plus but not required.

Compensation Range:
$50,000 - $55,000 USD
