  • Track completion of the Symptom Survey, complete appropriate follow ups, and escalate clinical concerns as needed.
  • Document social interventions, tasks, and care coordination activities in the electronic health record in a timely and standardized format.

WHAT WE ARE LOOKING FOR

  • Comfort with Technology. Comfortable using Slack, Zoom, Google Suite, and EHR systems to ensure accurate appointment scheduling and management and to support patients virtually.
  • Bias to action. Able to identify and prioritize needs and act immediately on urgent and important items.
  • Comfort with change and ambiguity. Comfortable working with limited direction and finding a path forward.
  • Strong relational skills. Able to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.
  • Organized & Communicative. Highly organized, detail-oriented, communicative with team members, and proficient in collecting and documenting patient clinical and demographic data in a timely manner.
